ADHD Assessments are offered through Dr. Hodges and are NOT covered by insurance due to the time required to complete a thorough assessment. The price for this assessment is $350 and includes the following components:

1. In person/Virtual Assessment that discusses symptomology in depth

2. Completion of D-REF to assess for executive functioning difficulties

3. Collection and Review of collateral data 

4. Diagnostic Write-up (upon review of assessment and collateral data)

5. Recommendation for services based on results

*Note: while there is no sliding scale available for this service, we do offer payment plans. When using the payment plan option, at least %75 of balance must be paid BEFORE assessment with the remaining $25% due at time of service. 

Scheduling: ADHD assessments are only scheduled on Thursday Afternoons and can be scheduled by reaching out to our office manager at admin@transformativetherapynashville. Please be sure to let her know you are interested in an ADHD assessment specifically so that you are scheduled for the appropriate appointment length. 

What to expect

• Completion of necessary practice paperwork including consent forms etc. 

• Assessment will last approximately 3 hours and should be in person when possible. 

• A link will be provided for you to complete D-REF assessment. This MUST be completed before assessment can be completed. You will be able to schedule your appointment before you complete the D-REF assessment however, if your D-REF has not been completed at least 48 hours before your scheduled appointment, your appointment will be canceled (there will be no fee associated with this cancellation)

• Assessment and associated measurement results will be reviewed and a diagnostic report provided within 10 business days of ALL components of assessment being completed (including collateral reports). 

ADHD assessments can be scheduled as a ‘one time service’ meaning that you do not have to participate in ongoing therapeutic services in order to complete assessment.
